For conscripts, recruits, or reservists:

  • Submit an application to the head of the territorial recruitment and social support center (TRC and SS) and state the circumstances of the loss.
  • Provide a color photo of 3x4 cm.
  • Present a passport.
  • Present the damaged document or its remnants (if any).

The decision to replace the military ID is made by the head of the TRC and SS.

For military personnel:

Submit a report to the commander about the reasons for the loss. After reviewing the report, a service investigation will be conducted. After its completion, the following documents need to be provided:

  • Report on the reasons for the loss of the military ID.
  • Passport.
  • A 3x4 cm photo without a corner.
  • The damaged document or its remnants (if any).
  • Extract from the order about the results of the service investigation (if the document was lost).

Replacement Timeline for Military ID

The document must be replaced within five working days from the date of registration of the application at the TRC and SS or their department.
